Accident Summary:
. AIRCRAFT HAD BEEN FLYING A ROUTINE COMBAT SUPPORT MISSION AND WAS TAKING OFF FORM ^CHI LANG^ ENROUTE TO ^HA TIEN^ WHEN THE ACCIDENT OCCURRED. THE WEATER WAS CLEAR AND VISIBILITY WAS GOOD ALTHOUGH SLIGHTLY SMOKEY. THE AIRCRAFT COMMANDER STATED THAT THEY HAD LANDED AT ^CHI LANG^ TO TAKE ON FUEL AND WERE CONTINUING ON TO ^HA TIEN^. THE PILOT, ^WO NELSON' PICKED THE AIRCRAFT UP FROM THE REFULEING PAD AND TURNED IT AROUND FOR AN EASTERLY DEPARTURE. AT THIS TIME THE AIRCRAFT COMMANDER STATED THAT HE MADE A HOVER CHECK WHILE THE AIRCRAFT WAS STABILIZED AT A HOVER AND FOUND THE RPM WAS MAINTAINED AT 6600. ^WO NELSON^ THEN BEGAN THE TAKEOFF. TRANSITIONAL LIFT WAS ACQUIRED BEFORE THE AIRCRAFT REACHED THE END OF THE AIRFIELD BOUNDRY. THE AIRCRAFT COMMANDER STATED AT 6600 RPM HAD BEEN MAINTAINED THROUGHOUT THE TAKE-OFF. AS THE AIRCRAFT NEARED THE AIRFIELD BOUNDRY AT AN AIRSPEED OF 30 KNOTS AND AN ALTITUDE OF 30 FEET, THE PILOT BROUGHT THE NOSE OF THE AIRCRAFT UP SLIGHTLY IN ORDER TO ENSURE THAT WIRES 15 FEET HIGH AT THE BOUNDRY FENCE WOULD BE CLEARED. AT THIS MOMENT THE AIRCRAFT COMMANDER NOTICED THAT THE RPM STARTED TO DROP AND HE TOOK CONTROL OF THE AIRCRAFT. HE IMMEDIATELY HOWERED THE COLLECTIVE PITCH IN AN ATTEMPT TO REGAIN OPERATIONAL RPM. AT THE SAME TIME THE AIRCRAFT COMMANDER NOTICED THE RPM DROP THE CREW CHIEF STATED THE AIRCRAFT SHUDDERED. THE AIRCRAFT COMMANDER'S ATTEMPT TO REGAIN OPERATIONAL RPM WAS UNSUCCESSFUL AND THE RPM CONTINUED TO DROP. HIS EFFORTS AT THIS TIME WERE TO KEEP THE AIRCRAFT IN THE AIR AS LONG AS POSSIBLE IN ORDER TO CLEAR AS MUCH OF THE CITY AS POSSIBLE. THE AIRCRAFT CONTINUED TO LOSE ALTITUDE AND THE CREW NOTICED SEVERAL TELEVISION ANTENNAS IN FRONT OF THEM AND THE AIRCRAFT COMMANDER STATED THE NOSE OF THE AIRCRAFT STRUCK A GUIDE LINE OF ONE OF THESE. AT THIS MOMENT THE CREW CHIEF STATED THAT THE AIRCRAFT HAD HIT THE TOP OF ONE OF THE BUILDINGS IN THE CITY. THIS CONTACT CAUSED THE AIRCRAFT TO TURN SHARPLY TO THE RIGHT APPROXIMATELY 90 DEGREES. THE AIRCRAFT CONTINUED DOWN IN THE SAME DIRECTION OFTAKE-OFF CRASHING INTO THE BUILDINGS, THE MAIN ROTOR BLADES STRIKING THE ROOF OF THE BUILDING ON THE RIGHT OF THE AIRCRAFT. THE AIRCRAFT CAME TO REST, ROLLED APPROXIMATELY 45 DEGREES ON ITS RIGHT SIDE IN A SMALL LOT SURROUNDED BY BUILDINGS, THE TAIL BOOM SUPPORTED BY A CONCRETE WALL. CLOSE EXAMINATION OF THE ACCIDENT SITE REVEALED THAT THE SKIDS OF THE AIRCRAFT WERE SEVERLY BENT AND THAT THE RED MAIN ROTOR BLADE WAS BENT UP OVER THE WHITE BLADE. I FOUND THAT THE AIRCRAFT DID NOT LOSE ANY DAMAGED PARTS DURING THE CRASH MOST PROBABLY DUE TO THE QUICK IMPACT AND LACK OF SKIDDING AT THE TIME.\\
