.The aircraft was in flight when an explosion and a bright flash came from the engine. An autorotation was made to the side of a canal. The right side of the aircraft was in three feet of water. Accident Summary:
. ^MR. WILCON^ HAD BEEN SENT OUT TO RUN UP THE AIRCRAFT AND ASSIST THE ARMAMENT PERSONNEL IN TESTING OUT THE FLEX SYSTEM OF THE AIRCRAFT. A NEW LEFT PYLON HAD BEEN INSTALLED AND IT WAS NECESSARY TO RUN THE AIRCRAFT UP TO TEST THE FLEX SYSTEM. ^MR. WILCOX^ WAS SITTING IN THE AIRCRAFT COMMANDERS SEAT. ^SP/4 CAROTENUTO^ WAS SITTING IN THE RIGHT REAR. ^SP/4 HUBBEL^ WAS SITTING IN THE LEFT REAR. AFTER THE INSTALLATION OF A NEW LEFT PYLON, ^MR. WILCOX^ WAS ASKED TO HOVER OUT OF THE REVETMENT SO THAT THE ROCKET POD AND MINI-GUN COULD BE BORESIGHTED. THE AIRCRAFT WAS FACING 080 PARALLEL TO RAMP ONE. ^MR. WILCOX^ PICKED THE AIRCRAFT UP AND STARTED TO HOVER TO THE LEFT (350) TO GET OUT OF THE REVETMENT. AFTER MOVING A FEW FEET THE RPM STARTED TO BLEED OFF. HE SET THE AIRCRAFT DOWN RATHER HARD. HE PICKED IT UP AGAIN AND THE TAIL STARTED TO SWING TO THE LEFT. (DOCKWISE) AT THIS POINT HE COMPLETELY RAN OUT OF LEFT PEDAL AND THE TAIL BOOM CONTINUED TO SWING TO THE LEFT (CLOCKWISE). FOR FEAR OF THE TAIL BOOM STRIKING THE EAST SIDE OF THE REVETMENT ^MR. WILCOX^ PULLED PITCH AND TRIED TO TAKE OFF OVER THE SOUTH SIDE OF THE REVETMENT TEARING OFF THE RIGHT SKID AND BREAKING THE CHIN BUBBLE OF THE AIRCRAFT. THE AIRCRAFT CONTINUED TURNING AND BY THIS TIME HAD TURNED A FULL 180 AND WAS FACING DUE WEST. ^MR. WILCOX^ THEN DECIDED TO TRY AND GET THE SHIP AIRBORN. HOWEVER, BY THIS TIME THE RPM HAD BLED DOWN TOO FAR AND THE WIND (05 KNOTS AT 135) WAS COMING FROM HIS LEFT REAR. THE SHIP CONTINUED TO BLEED DOWN AND ^MR. WILCOX^ TURNED TOWARD THE BANK TO TRY TO KEEP FROM GOING DOWN IN THE WATER. THE SHIP STRUCK THE EDGE OF THE BANK JUST OUT OF THE WATER. THE TAIL BOOM ITSELF WAS HANGING IN THE WATER, AND PART OF THE ARMAMENT SYSTEM WAS IN THE MUD AND WATER AT THE EDGE OF THE BANK. THE AIRCRAFT REMAINED UPRIGHT, WITH ENGINE RUNNING AND BLADES TURNING. ^MR. WILCOX^ THEN NOTIFIED THE TOWER AND THE ^175TH AVN CO^ OPERATIONS OF WHAT HAD TAKEN PLACE. HE THEN COMMENCED TO SHUT DOWN THE AIRCRAFT. THE PASSENGER HAD BY THIS TIME DEPARTED THE SHIP.\\
