Accident Summary:
.The aircraft was on a firefly mission for Vinh Long sector and was taking off on his second flight at 2230. The aircraft made a right turn out of traffic, proceeded around the Vinh Long Airfield to the east and turned right again to parallel the runway heading 260 degrees approximately 400 meters south of the runway. The crew heard a loud bang and the aircraft yawed to the right violently. 1LT Sawyer lowered pitch to regain control. He attempted to level the aircraft by applying collective in hopes of being able to land on the runway. The aircraft again yawed right and the nose dropped down. 1LT Sawyer then realized he had lost his anti-torque system and lowered pitch and continued down into the paddy south of the runway. He flared and attempted a hovering autorotation as the aircraft turned to the left. He hit heavy on the left skid in approximately two feet of mud and water. The loud bang was the 90 degrees gear box and tail rotor severing from the vertical fin and flopping over to the right side of the vertical fin with one tail rotor blade embedding into the vertical fin causing complete tail rotor failure. No maintenance deficiencies were found and supervisory error was a contributing factor in that little if any training is conducted at night for emergencies to include autorotations
