Accident Summary:
. BEFORE THE ACCIDENT OCCURRED THE AIRCRAFT WAS BEING TEST FLOWN. AIRCRAFT WAS RUN UP ON THE GROUND BY MAINTENANCE PERSONNEL. PURPOSE OF THE RUN-UP WAS TO CHECK THE TRACK OF THE MAIN ROTOR BLADES, WHICH WAS BEING PERFORMED BECAUSE OF A SLIGHT LATERAL VIBRATION FOUND DURING THE DAY'S FLIGHT. IT WAS THOUGHT THAT THIS MIGHT BE THE UNDERLYING CAUSE OF THE LATERAL VIBRATION WHILE INFLIGHT. THE BLADES WERE FOUND TO BE IN PERFECT TRACK. THE TAIL ROTOR WAS NOT CHECKED BECAUSE THERE WAS NO HIGH FREQUENCY VIBRATION EXPERIENCED. WO CASTILLO, PILOT OF THE AIRCRAFT, PRIOR TO THE TIME OF THE ACCIDENT, HAD JUST RETURNED FROM ANOTHER FLIGHT. HE WAS ASKED TO TAKE AIRCRAFT 844 UP FOR A TEST FLIGHT TO SEE IF THE LATERAL VIBRATION STILL EXISTED. WITHOUT SHUTTING THE AIRCRAFT DOWN MR. CASTILLO CLIMBED INTO THE PILOTS SEAT, SPEC NASH, A MEMBER OF THE LINE CREW RODE IN THE FRONT SEAT AS A TECHNICAL OBSERVER. MR. CASTILLO DID NOT SHUT THE AIRCRAFT DOWN AND MAKE A PREFLIGHT. AN EAST DEPARTURE WAS MADE FROM VINH LONG AIRFIELD, ALL SYSTEMS NORMAL AND OPERATING PROPERLY. ONCE AIRBORNE THE AIRCRAFT WAS PUT THROUGH VARIOUS MANEUVERS, NORMAL CRUISE FLIGHT, NORMAL TURNS AND A HOVER OUT OF GROUND EFFECT. UP TO THIS POINT NO LATERAL VIBRATION WAS EXPERIENCED. THE PILOT THEN NOSED THE AIRCRAFT OVER TO REGAIN HIS AIRSPEED AND TRIED A FEW STEEP TURNS, APPROXIMATELY 90 DEG BANKS TO THE RIGHT AND LEFT, STILL NO VIBRATIONS, HE THEN ENTERED A SHALLOW DIVE, ABOUT 20 DEG, THE AIRSPEED BEGAN TO BUILD. AS THE AIRSPEED APPROACHED 120 KNOTS THE LATERAL VIBRATION BECAME APPARENT. AT THIS TIME THE PILOT BEGAN HIS PULL OUT BY ADDING AFT CYCLIC AND TWO TO THREE POUNDS OF TORQUE TO MAINTAIN ROTOR RPM IN THE GREEN. BY THIS TIME THE AIRCRAFT HAD ATTAINED 140 KNOTS OF AIRSPEED, 1500 FEET OF ALTITUDE AND THE LATERAL VIBRATION HAD BECOME EXCESSIVE. THE PILOT STATES HE WAS EXTREMELY GENTLE ON THE CONTROLS DUE TO THE VIBRATION. AT THIS POINT HE HEARD A LOUD NOISE TO THE REAR OF THE AIRCRAFT, THE ENGINE SURGED, THE AIRCRAFT YAWED TO THE RIGHT ABOUT 70 DEG, HE TRIED TO ADD LEFT PEDAL TO TRIM THE AIRCRAFT UP BUT THE PEDAL WENT TO THE FLOOR WITHOUT RESISTANCE. MR. CASTILLO ENTERED AUTOROTATION. AT THIS POINT HE HEARD A LOUD NOISE TO THE REAR OF THE AIRCRAFT, THE ENGINE SURGED, THE AIRCRAFT YAWED TO THE RIGHT ABOUT 70 DEG, HE TRIED TO ADD LEFT PEDAL TO TRIM THE AIRCRAFT UP BUT THE PEDAL WENT TO THE FLOOR WITHOUT RESISTANCE. MR. CASTILLO ENTERED AUTOROTATION. AT THIS TIME THE AIRCRAFT STREAMLINED AND WAS HEADED DUE NORTH. THE PILOT STATES HE SAW AN UNOBSTRUCTED RICE PADDY TO HIS LEFT AND TURNED THE AIRCRAFT IN THAT DIRECTION. HE THEN BEGAN SLOWING THE AIRSPEED, AT APPROXIMATELY 10 TO 15 FEET ABOVE THE GROUND HE PULLED INITIAL PITCH TO FURTHER SLOW THE AIRSPEED AND RATE OF DESCENT. AS COLLECTIVE PITCH WAS APPLIED THE AIRCRAFT YAWED TO THE RIGHT AND TOUCHED DOWN WITH THE RIGHT REAR SKID AT APPROXIMATELY 10 KNOTS FORWARD AIRSPEED. IT THEN ROLLED TO THE LEFT ON ITS SIDE AND ROTATED IN A CLOCKWISE MOTION. IT CAME TO REST WITH THE NOSE POINTING APPROXIMATELY 010 DEG AT COORDINATES XS090415, RVN.\\
