Accident Summary:
. ON THE MORNING OF 3 DEC 71, THE CREW OF AIRCRAFT ^69-15882^ MADE A THOROUGH PREFLIGHT INSPECTION IN PREPARATION FOR THE DAY'S MISSION. THEIR MISSION FOR THAT DAY WAS THE SUPPORT SHIP FOR PHONG DINH PROVINCE. THE AIRCRAFT DEPARTED VINH LONG AAF AT 0815 HOURS AND ARRIVED AT THEIR DESTINATION, CAN THO AAF, AT 0830 HOURS. THE AIRCRAFT WAS THEN FLOWN THROUGHOUT THE PROVINCE TO THE VARIOUS DISTRICT TOWNS AND ADVISORY TEAMS CARRYING SUPPLIES AND MAIL. AT APPROXIMATELY 1500 HOURS, AFTER THE AIRCRAFT HAD FLOWN FOR 04+00 HOURS, AND HAD MADE TWENTY-FIVE TAKEOFFS AND LANDINGS,TTHE MISHAP OCCURRED WITHOUT PRIOR WARNING. THE AIRCRAFT WAS DEPARTING THUAN NHON, THE LOCATION FOR ADVISORY TEAM 56, WITH THE PILOT AT THE CONTROLS. AT APPROXIMATELY 30 FEET OF ALTITUDE AND 5 KNOTS OF AIRSPEED, A LOUD SNAPPING SOUND WAS HEARD AND THE AIRCRAFT BEGAN TO SPIN TO THE RIGHT. THE AIRCRAFT COMMANDER IMMEDIATELY TOOK THE CONTROLS AND DISCOVERED THAT THE ANTI-TORQUE PEDALS HAD NO EFFECT AND THAT APPROXIMATELY ONE AND ONE HALF REVOLUTIONS TO THE RIGHT AND STARTED TO SETTLE IN WITH THE AIRCRAFT COMMANDER MAINTAINING A LEVEL ATTITUDE WITH THE CYCLIC. AS CUSHIONING PITCH WAS APPLIED BY THE AIRCRAFT COMMANDER, THE RIGHT SKID CAME IN CONTACT WITH A SMALL WOOD AND METAL STRUCTURE THAT CAUSED THE AIRCRAFT TO ROLL ON ITS LEFT SIDE. THE RETREATING BLADE OF THE MAIN ROTOR SYSTEM ST*UCK THE GROUND, RIPPING THE MAIN TRANSMISSION OUT OF ITS MOUNTS AND DRIVING IT FORWARD INTO THE TROOP CARRYING COMPARTMENT. THE AIRCRAFT REMAINED ON ITS LEFT SIDE. THE CREW AND THEIR ONE PASSENGER EXITED OUT OF THE RIGHT SIDE OF THE AIRCRAFT.\\
